Suzanne Ryer

Suzie is the Senior Project Coordinator in Senior Services at Advocate Health. She is a physical therapist who is board certified in Geriatrics with nearly 20 years of clinical experience in a variety of clinical settings. Her focus is on interdisciplinary geriatric care with an emphasis on fall prevention and emergency department care. She has led efforts to create Geriatric emergency departments in a large health system and successfully integrate physical therapy as part of the interdisciplinary team in this setting to improve patient outcomes. Suzie has created interdisciplinary fall prevention and mobility programs to improve clinical outcomes and combat hospital-associated disability and readmission. Suzie is the co-chair of the Milwaukee County Falls Prevention Coalition and has created partnerships with a variety of community-based organizations at both the state and local levels. Suzie serves on the Executive Council for the Southeastern Regional Trauma Advisory Council and on the steering committee of the Falls Free WI Coalition and co-leads workgroups in both. Suzie has published multiple articles and presented nationally on fall prevention and geriatric emergency care.
Ann Gallo

Ann is the Senior Services Program Coordinator for Advocate Health. Since 2016, Ann has worked with Senior Services on the implementation of geriatric models of care, geriatric education, tools, and resources to improve care for older adults across the Advocate system. Ann co-leads the Age-Friendly Health System implementation across twenty-five hospitals. Ann also leads the development and implementation of system Dementia Resource Training, system Geriatrics for Advanced Practice Education, and national Geriatrics Models of Care Collaborative educational series. Ann is the co-chair of the Milwaukee County Falls Prevention Coalition, co-chairs the State Falls Prevention Collaborative, and State Falls Prevention Best Practice workgroup, and is a steering member of the Falls Free WI State Falls Prevention Coalition. Ann also serves on the Executive Council for the Southeastern Regional Trauma Advisory Council and is the co-chair of the injury prevention committee.
