About

The Milwaukee County Falls Prevention Coalition (MCFPC) is an interprofessional group of experts, advocates, and community members collaborating to prevent falls in the adult community by improving access to programs, interventions, and resources to help reduce the risk of falls.


The Milwaukee County Falls Prevention Coalition (MCFPC) will address the adult community’s risk of falling by providing access to comprehensive programs and targeted interventions. The coalition will create links between community programs and services to enhance collaborative efforts. Milwaukee County will be a national leader in the prevention of falls in the adult population.

Mission


  • Access: Increase access to fall screening and fall prevention interventions in all communities in Milwaukee County.

  • Awareness: Create broad awareness of fall prevention within the community through branding, community education & communication plan.

  • Collaboration and Member Engagement: Increase collaboration through improved handoffs amongst community partners and strengthening membership engagement within the coalition.

  • Data and Metrics: Capture data to monitor the success of fall prevention initiatives and to identify patterns and prevent falls within the community.

  • Education: Educate providers on evidence-based screening & interventions & provide access to resources to support the community.
